This is a common occurrence in house up here. This is a broken bathroom fan duct that has been releasing warm air into the attic and has caused an ice build up that will cause problems in the spring when it melts.

This is a ventilation baffles. This is what maintains the air flow over your new insulation. In a good application of insulation you would not be able to see the bottom of the baffle. This one does not have enough insulation under the baffles to help maintain the R-value in this area.
Here is another bathroom fan duct that was leaking warm air into the attic and causing the moisture to build up in the insulation form ice and then melt every time the fan was turned on.

This is a view of the Arctic Hatch that we custom build for each house that has one installed. They have a vapor barrier and weather stripping along with an R-30 R-value.
